Episode 1 IC Fire-Themed Hauling Race Immunity Idol Drake I think a cannon counts as "fire-themed."  If you don't, it's not my problem.
Episode 2 RC Hauling Map Piece, Tribal Looting Drake Retrieving various items (including a treasure chest) from underwater hiding places.
Episode 2 IC Haul-Swim-Dig Immunity Idol Drake  
Episode 3 RC Sink That Boat Map Piece, Tribal Looting, Luxuries Drake Luxuries were blankets and such.
Episode 3 IC Buck-Buck Immunity Idol Drake Variant: The weight being held up is the smallest tribe member, and there is some pretense of this being a team challenge in that everybody is holding them up via pulleys and ropes, but every 5 minutes a person from each tribe has to let go, so the challenge ultimately falls on the person they've placed in the back (who is, naturally, their strongest player).
Episode 4 RC Dig Up Stuff Map Piece, Tribal Looting, Luxuries Drake Luxuries = a sewing kit.
Episode 4 IC Attack Zone Game Immunity Idol, Tribe Member Morgan (The Tribe Member theft was a surprise not announced until the challenge was already decided.)  In this version of the game, there is no single "attack zone"; instead, players are taking turns moving across a web of balance beams, and whenever they encounter somebody else, one must force the other into the ocean.  This would have been really fun to watch if Drake hadn't blatantly thrown the challenge.
Episode 5 RC Division of Labor Race Map Piece, Tribal Looting, Luxuries, Survival Equipment Morgan Luxuries: a portable shower, sponges, shampoo, etc.  Survival Equipment: cooking pot.  This is sort of hard to categorize, given the way the challenge is timed, but I do think it qualifies as a DOLR.
Episode 5 IC Gross Food Challenge Immunity Idol Morgan A "Wheel of Food" that determined the ingredients of Jeff's Seafood Smoothies.  Fun!  I've heard that the production team was stumped for this season because it turned out that neither Panamanians nor pirates really ate anything sufficiently gross, so this generic "nasty things from the ocean" game was cooked up.
Episode 6 RC Target Game Map Piece, Tribal Looting, Big Feast Drake Tribes operated cannons to shoot targets.  Not as awesome as it should have been.  The Big Feast was a 
Episode 6 IC Buck-Buck Immunity Idol Morgan A fabulous variant in which each tribe has three weight-bearers (two chosen by the tribe, one chosen by the other tribe for them), and the tribes take turns adding weights to the opponent weight-bearer of their choice.  Great, great TV.
Episode 7 RC Escape the Cage Freedom From/For Outcasts Outcasts aRHRHHHGHGHGHG.

Episode 8 IC Diving Immunity Cutlass Burton (Given to Rupert) Sort of a horizontal diving challenge - the Survivors went underwater and had to pull themselves along a rope that ran the length of a substantial wooden platform out in the ocean. 
Episode 9 RC Target Game Big Feast Rupert (Given to Burton, Shared w/ Lil) Slingshot game.  The trip was to a small island (referred to by CBS in an obvious slip of the tongue as a 'remotely beautiful location') for a substantial breakfast. 
Episode 9 IC Coconut Chop Immunity Cutlass Rupert The topic was pirate lore.  
Episode 10 RC Paired-Up Obstacle Course Elaborate Trip Burton (Given to Jon) and Lil Elaborate trip was a ride on a fancy motorboat, plus pizza and enough beer to get Lil to declare that she was the king of the world, or something like that.
Episode 10 IC Projectile Elimination Immunity Cutlass Burton The famed "Killer" blowgun game, the second elimination-style IC of this season.
Episode 11 RC Match Your Family's Answer Family Visit, Screw Rest Of Tribe Jon & Thunder Dan "She died, dude."  With these simple words the world was introduced to the greatest castaway of all - Thunder Dan, or, as I misheard it at the time, "Thugga D."  Oh, if only he could have replaced Lil and served out a longer sentence on La Isla Balboa!  Minor note: for the first time that I remember, a "family" challenge was made elimination-style: correct answers meant you could send someone else's family member down a plank and eventually out of the game.
Episode 11 IC Word Game Immunity Cutlass Darrah Anagrams / "Word Whomp" type game developed by a youthful fan of the show with suspicious family connections to CBS higher-ups.  Anyway, the Survivors had to find as many legitimate, correctly-spelled words as possible out of a longer phrase.  Nearly ruined, Survivor 3 Fallen Comrades style, by Jeff's inability to correctly grade the stupid things.
Episode 12 RC Oceanic Obstacle Course Elaborate Trip Darrah, Jon, Lil Winners got a day away at a fabulous spa hotel - shower, massages, dinner, and good night's sleep type of thing.  If memory serves me, this is the first teams-of-3 challenge we've seen.
Episode 12 IC Target Game Immunity Cutlass Darrah Shooting at stuff with flare pistols done up to resemble muskets.  Like the cannon challenge, just not as cool as it could have been.
Episode 13 RC Super Challenge Car, Elaborate Trip Burton (Shared w/ Jon) Oh boy.  The GMC Envoy "XUV."  I cannot wait to drive one of these.  Gag me with a spoon.  The Elaborate Trip was a boat trip, dinner, and overnight stay in the old part of Panama City, with a ride in the Car on the way.  Oddly, the Car was not mentioned at the challenge itself to the other Survivors.  Also oddly, they let Burton drive through the city to the reward site.  I'm surprised they didn't try to sneak in a little shopping spree.  Where were the spoilers on this one??
Episode 13 IC Balance and Race / Fill Something Immunity Cutlass Darrah You had to run along a balance beam to fill a series of little containers so that keys would float to the top.  This sort of clever hybrid game was ruined as soon as Darrah discovered that, rather than physical or mental ability, this game actually tested skinniness ability.  Outwit...Outplay...Outstarve.  (Listed elsewhere as Fill Something.)
Episode 14 IC Survival Trivia Immunity Cutlass The Jury Aside from the inane twist of letting the jury play (which amounts to a "Nobody Wins Immunity" twist, since the jury has five heads rather than one), this was one of the best trivia games ever, if only for the wonderful question "What's the best way to avoid a shark attack," to which only one Survivor correctly answered "Stay out of the water."  I really love these people.
Episode 14 IC Balance Endurance Immunity Cutlass Lil Basically a repeat of the post-merge challenge from Marquesas.  A good one, though.
